3.0e Core Books

Player's Handbook

None of the base races in the Player's Handbook have level adjustments or racial hit dice. Their character level and ECL are both equal to their total class levels. Their creature types and subtypes are not given in the PHB, but can be found in the relevant sections of the Monster Manual, except for humans.

Dungeon Master's Guide

The DMG offers no canon playable races. It does provide a "half-human" and a "gold dwarf", but only as examples of homebrewed races in Chapter Two, which is dedicated exclusively to variant, alternate, and homebrewed rules.

3.0e Monster Manual I

When D&D 3.0 was first launched, the terms "Level Adjustment" and "Effective Character Level" did not exist. Instead, there was a "Level Equivalent" system, which functioned much like ECL except that it did not affect the amount of experience required to level up. As a result, while a Tiefling or Aasimar would have to start at level 1 in a party of otherwise level 2 players, this level gap would quickly evaporate, leaving high-LE characters at a long-term advantage. It was also common, in the beginning, for monster races to have most of their stats printed in one book while their LEs or LAs would be printed in a separate book, because WotC was using massive amounts of drugs at the time and had no idea what they were doing, which also explains the Magic: The Gathering Sixth Edition rules. The following monster races had their Level Equivalents printed in the DMG and all of their other stats printed in the Monster Manual. Subraces such as Duergar, Drow, and Svirfneblins were not given level equivalents and thus remained unplayable.

3.0e Supplements

2001 June - Forgotten Realms Campaign Setting

The FRCS replaced the old "Level Equivalent" system with "Level Adjustment" and "Effective Character Level", wherein characters would level up at a rate that was appropriate for their character's overall power, rather than just their class levels. This prevented races with level adjustments from rapidly becoming too powerful as they gained experience. None of the playable races had racial hit dice, and none of the monsters had level adjustments for adapting them into playable races, so no creature had both racial hit dice and a level adjustment. The setting's Shield Dwarf and Moon Elf are functionally identical to the Hill Dwarf and High Elf from the core books.

2001 August - Manual of the Planes

MotP was the first non-core book to experiment with giving Level Equivalents to NPC monsters. It did so for six of them. It provided a definition of level adjustment that seemed to include RHD, yet some of the level adjustments that it gave were lower than the RHD of their associated races, implying that LA did not include RHD.

2001 October - Oriental Adventures

Oriental Adventures was the first book to mix fully playable races with pseudo-playable monsters whose level adjustments would be given in a single table far away from all the other monster stats. The same table also provided level adjustments for a bunch of monsters from the Monster Manual I who had already gotten such a table in the DMG, because everyone at WotC was abusing massive amounts of heroin during this time period. They have been excluded here. Several playable races from the PHB got tweaks as well. OA, like the Manual of the Planes, provided a definition of level adjustment that seemed to include RHD, yet some of the level adjustments that it gave were lower than the RHD of their associated races, implying that LA did not include RHD.

The Epic Level handbook was a pile of jibbering nonsense in which some monsters had level adjustments in the negative twenties. The only monster in the book that would be considered playable by an even remotely sane person was the Mercane.

2003 February - Savage Species

Despite its reputation as the go-to book for making monstrous races playable, Savage Species offered only one new fully playable monster race, and reprinted three "you can certainly try" monsters. This was the first book in which "Level Adjustment" definitely had its current meaning, in which levels from racial hit dice are considered a separate thing and not included in LA.

3.5e Monster Manual I

The 3.5e revision of the Monster Manual included full PC stat blocks for a fuckton of its subraces and monsters, making it a huge improvement over the 3.0 MM1 and MM2. About twice as many monsters only got level adjustments, and, in another huge improvement over the 3.0 core books, those LAs were incorporated into their NPC stat blocks instead of being shoved off into a table in the DMG. For the sake of brevity, only races with full PC stat blocks will be included from now on.

2003 August - Dragonlance Campaign Setting

Dragonlance was notable for not giving two shits about what stats any race had in any other setting. It knocked minotaurs down from a race with 6 RHD and a +2 level adjustment to having zero of both, which was all kinds of fuckawesome for people who wanted to play as minotaurs in low-level campaigns, and also made up its own stats for gnomes, half-ogres, and God-Emperor knows what else.

Book of Exalted Deeds

BoED only had one fully playable race, but it gave us a bunch of Archons and Guardinals and Eladrins and shit as NPCs with level adjustments who are just too cool to leave out. The Bariaur here is referred to as simply "Bariaur" in BoED, but the Planar Handbook clarifies that this version of the Bariaur is more properly referred to as the "Exalted Bariaur", and that simply "Bariaur" should be reserved for the Bariaur in the Planar Handbook and Manual of the Planes. No advice was offered on the Dragonlance Minotaur situation.
