There is no true 'structure' in the Cult. When Rakdos, the demonic founder of the cult, isn't napping in pools of searing-hot magma for decades on end, high-ranking Bloodwitches vie for power.
 
Honestly, the best way to describe what happens is to compare it to that one college fraternity that routinely throws wild parties, crank the violence and debauchery up to a million, deck everyone out in bladed chains, spikes, black leather, and dump an ocean's worth of blood on everything.
