I have mixed feelings about the artwork
In most role-playing books all the artwork are drawings but in this book the artwork is all photographs of models (sometimes nude) with stage makeup and Photoshop.
It looks really good but it is not what I am used to.   
Homosexuality: what abilities change by role-playing a LGBT character?
None. There are no abilities, feats, skills, spells or anything that a LGBT character can or cannot have.     
I really like how all alignments still have sex but is how each alignment peruses sex that determines the characters specific alignment.
I did not like the 7th ability Appearance (App) the skills that (App) modifies are Disguise, Bluff, Gather information, and Intimidate. I can find no difference between (App) and (Cha) the players hand book dose a good job explaining how someone can have high Intelligence and low Wisdom but how can someone have high charisma but low Appearance? 
Sexually Transmuted Diseases
The whole reason why I got this book is because a character in one of my campaigns wanted to play a prostitute saying “I can get gold to do what I like doing, there is no downside” as a DM I need to take her fun away
STDs are treated the same as other magical diseases listed in the DMG and can be treated the same way. {will have list uploaded later}
