Nilshai, also known as Ethereal Theurges, are a species of trilaterally symmetrical, worm-like aberrations who inhabit the Ethereal Plane. Powerful mages, they regularly invade the material plane to take humanoid slaves.
They first appeared in Dungeons & Dragons 3rd Edition, making their debut in the "Unapproachable East" splatbook for Forgotten Realms, and were then expanded upon in the 3e splatbook Lords of Madness.
